The cover features an illustration of Ida Lewis standing with Lime Rock Lighthouse in the background, as well as the rowboat "Rescue". Music contains "Rescue Polka Mazurka" with lyrics, the title of the work being "The Ocean Waves Dashed Wildly High." Published by the Cory Brothers of Providence. The back cover has handwritten music and lyrics in pencil. A tear in the front and back cover was sewn shut with thread. Scotch tape is present on back and front covers as well.
